Feeding Questionnaire
If you have concerns about your child’s feeding, this Feeding Matters Infant and Child Feeding Questionnaire© is a great place to start. Early detection and treatment of feeding problems are critical to the long-term health and well-being of affected children. This is an evidence-based tool which may be used to promote early identification of feeding disorders.
You can take your completed Questionnaire to your child's Primary Care Physician in order to request a referral for feeding therapy with AID-L.
